ENROLLMENT INTAKE & CONSUMER ASSIGNMENT WORKFLOW

All potential consumers will be linked to Dedicated Care Health Services by phone contact with the Department of Behavioral Health’s Access Help Line, (1 - 888 - 7WE-HELP or, 1 - 888 -793- 4357).

ALL walk-in consumers shall also use the access line to receive services from Dedicated Care Health Services.
Staff will work with the consumer to go through the process.
Coordination of delivery is ensured by the Clinical Director and assured by the QA Director.

OUR PROCESS
Step 1

INTAKE PROCESS: (Taking referrals, gathering and verifying information)

  1. Receive Referrals: Scheduled in Credible; Walk-ins AHL linked (if schedule permits); Phone calls (schedule appointments)
  2. Gather all necessary information
  3. Log the referral
  4. Verify insurance benefits
  5. Determine eligibility of referral
Step 2

ENROLLMENT PROCESS: (Accepting consumer and ensuring that consumer completes/signs all necessary documentation.)

  1. Set-up consumer in EMR systems.
  2. Explain Services and Benefits to consumer.
  3. Complete all necessary documentation in Credible.
  4. Prepare Consumer Packet and ensure Consumer signs all necessary documentation.
Step 3

CONSUMER ASSIGNMENT PROCESS BASED ON VALID INITIAL 30-DAY AUTH: (MHRS Office). Upon completing intake:

  1. Consumer will be assigned a Case Social Worker (CSW)based on consumer’s preferences and in consultation with Clinical Managers.
  2. Consumers will be assigned a Therapist after consultation with consumer and Clinical Manager.
  3. Based on need, Consumer will be referred to be scheduled for psychiatric evaluation and medication management (Psychiatrist appointment).
  4. Hard-to-engage consumers by Intake Coordinator or CSW will be referred to Outreach Team.
  5. Discharge of Consumer will be based on the 3-2-1 approach and upon endorsement by Clinical Manager.
